Medical Release and Tuition Agreement Form
Currently, registration can be done either in person at Lone Mountain or over the phone, but we recommend on site enrollment. Please be prepared to spend a few minutes registering your child, the process of which has three goals:
1. To place your child in the most appropriate class
2. To update all family information and complete necessary paperwork
3. To educate families about policies, procedures and to help them feel comfortable and welcome
Payment is due at the time of enrollment.
For your child's safety, classes have a maximum number of children that can participate. Class openings change very quickly and may not be available if you hesitate on registering your child. If you are unable to get into a desired class, the front desk staff can place your child on a wait list and will notify you as soon as there is an opening.
